5 % fascií jsou buňky

Naše buňky jsou obsaženy ve fasciích. Buňky se v nich drží pohromadě jako suchý zip (Mayers 2013) na teniskách malého dítěte. Nejčastěji vyskytující se fasciální buňkou jsou fibroblasty, avšak pojivová tkáň obsahuje i další. Jaké? Viz tabulku dole:

V tabulce vidíš obrovskou rozmanitost materiálů, které jsou schopny vzniknout z omezeného množství vláken a různých dalších prvků. Jsou seřazeny od nejpevnějších po nejtekutější. (Mayers 2013)

Ano, vše co je v tabulce a ještě o něco více je pojivová tkáň, jen za každým trošku jinak složená. A ty už víš, že buňky jsou součástí mezibuněčné hmoty, do které patří ještě tekuté prvky (proteoglykany) a kolagenová elastinová a fibrinogenní vlákna a další veci. A touto kombinací tvoří schopnost ,,suchého zipu“.

Tělo se skládá z různých částí, ale všechny se skládají jen z několika prvků.

Všechny výše zmíněné typy struktur se od sebe odlišují typem, množstvím vláken a tekutých prvků. Jsou zabaleny do lepkavé tekuté základní hmoty, která vytváří síťovinu, která se neustále přizpůsobuje vnějším i vnitřním podmínkám.

Zaměřme se však na buňky. Fibroblasty a žírné buňky vytvářejí svou aktivitou pojivové tkáně, které se přizpůsobují změnou poměru vláken, lepkavých proteoglykanů a vody.

Jaký je smysl života fibroblastů?

Smyslem života fibroblastu je práce, práce a zase práce (komunisté by byli hrdí z takového pracanta). Je však důležité pochopit, že fibroblast má kámoše roboše. Těmi kámoši jsou fibrocyt a fibroklast. Áno je to, totéž jako u kostí – osteoblast, osteocyt, osteoklast. Že jsi o to nikdy neslyšel? Vysvětlím…

Mnoho buněk lze rozdělit do tří kategorií:

  • blast – tvoří novou tkáň
  • cyt – udržuje a stará se o existující tkáň
  • klast – „ničí, požírá“ staré tkáňe (Med 2018)

A ty si můžeš doplnit, co to bude za buňku: fibroblast, osteoklast, chondoblast atd.

Stejně jako v komunismu je nahoře někdo, kdo řídí práci na budování lepších zítřků. Jinými slovy, tělo samo zaměstnává „roboše“ a „architekty“. Kdo je však tento architekt? Architektem lidského těla je proces zvaný mechanotransdukce.

Každá konstrukce, a tou naše tělo je, potřebuje někoho, kdo řekne, jak se má co dělat, a někoho, kdo ty věci udělá.

Mechanotransdukce architekt fascií

Z názvu vyplývá, že se jedná o proces, při kterém je určitý druh mechanické energie přenášen/převáděn na buňky (Ingber 2008), které na různé druhy mechanické energie reagují různými způsoby. Když je fasciální tkáň vystavena nárazu, vibracím, tlaku nebo stresu, vytvoří se v matrici tkáně elektrický náboj (piezoelektrický náboj) (Becker 1998), který způsobí, že fibroblastové buňky přejdou na dané místo a začnou produkovat kolagen a další molekuly podle potřeby podél těchto stresů, vibrací a šoků.

Jinými slovy, fibroblasty cestují po těle a „serou“ tam, kde je jich zapotřebí. Posílají se podle mechanického zatížení, které na tělo působí. To, co sere, jsou dlouhá nebo krátká (podle potřeby) kolagenní vlákna typu I a III (nejčastěji) nebo tekuté prvky, jako jsou proteoglykany. Místo, kde vykonávají svou „potřebu“, nejčastěji způsobuje ztuhnutí „svalů“.

Piezoelektrický náboj určuje směr fibroblastů. A v závislosti na požadavcích prostředí, do kterého se dostanou, začnou vylučovat buď kolagen, nebo proteglykany.

Ale ty už víš, že svaly netuhnou, ale fascie. A hlavním viníkem jsou fibroblastové buňky, které v intersticiální hmotě produkují převážně kolagenní vlákna reagující na mechanickou zátěž. A co hůř, fibroblasty do tebe nejen „serou“ kolagen, ale také „močí“ proteoglykany, které tvoří přizpůsobivé „lepidlo“, jež drží vše pohromadě.

Komunikačný systém ECM

Fibroblasty jsou součástí komunikačního systému ECM. Jsou nezávislé a fungují paralelně s centrálním nervovým systémem (Parisi a Allen 2019). Fibroblasty jsou v této „telekomunikační síti“ anténou, přijímačem a reproduktorem.

Podobně jako elektromagnetické záření v naší telekomunikační síti funguje extracelulární matrix ve fascii.

Biomechanická signalizace, která řídí tyto změny tkání na buněčné úrovni, vědcům pouze prozrazuje jejich tajemství, ale důsledky této nové mechanobiologie jsou dalekosáhlé pro všechny manuální a pohybové terapeuty. Každá buňka, a zejména každý fibrocyt, nejen ,,ochutnává“ své chemické prostředí, ale ve svém prostředí (Candace Pert et al. 1997) ,,naslouchá“ a reaguje na mechanické prostředí tahu a také stlačení.

Další forma fasciální signalizace vychází z myšlenky, že vlhká kolagenová síť tvoří tekutý krystal – polovodivou síť. Tlak nebo napětí vytváří v této tkáni iontový tok, známý jako piezoelektřina, a tento elektrický tok stimuluje nebo stlačuje fibroblasty k tvorbě (nebo netvorbě) nových vláken (Earls a Mayers 2017).

Biomechanická stimulace pojivové tkáne

Robert Shleip: ,,Biomechanická stimulace je pro zdraví tkání přinejmenším stejně důležitá jako biochemické prostředí. Bez správné mechanické stimulace totiž fasciální fibroblasty nevytvoří adekvátní fibrózní matrix bez ohledu na to, jak dobré či špatné je jejich biochemické prostředí. Zatímco nutriční péče může zlepšit biochemické prostředí, sportovní a pohybové terapie jsou mocnými nástroji pro podporu optimální biomechanické stimulace pro chování fibroblastů při remodelaci matrix. Fibroblasty jsou vybaveny mnoha zařízeními pro vnímání tahové a mechanické smykové stimulace, která na ně působí. V reakci na to neustále mění svou metabolickou funkci.“

To tedy znamená, že pokud jsou pojivové struktury správně zatěžovány, fibroblasty přizpůsobují svou činnost remodelace ECM, takže architektura tkáně ještě lépe reaguje na každodenní požadavky. Mění se nejen hustota kostí, ale také fasciální tkáně, které reagují na dominantní způsob zatížení.

Při správné aplikaci tréninkové zátěže můžeme přetvořit ECM tak, abychom dosáhli podobných změn jako u Dominiky.

S pomocí fibroblastů pomalu, ale vytrvale reagují na každodenní zátěž i na specifický trénink (Kjaer et al., 2009). Jejich remodelační aktivita reaguje zejména na opakované narušení mechanické integrity okolní matrix. Požadavky na pevnost, roztažitelnost a smykovou kapacitu tkáně stimulují fibroblasty k reakci v procesu neustálé rekonstrukce a remodelace fasciální sítě.
